Part C Indicator Descriptions

Indicator 1: Timely Service Delivery

Percent of infants and toddlers with Individual Family Service Plans (IFSP) who receive the early intervention services on their IFSPs in a timely manner.

Indicator 2: Settings

Percent of infants and toddlers with IFSPs who primarily receive early intervention services in the home or programs for typically developing children.

Indicator 3: Child Outcomes - Outcome 1 (O1)

Positive social-emotional skills including social relationships

  • Summary Statement 1 (SS1): Percentage of children who changed their growth (pattern) while in the Birth to 3 program.
  • Summary Statement 2 (SS2): Percentage of children who were functioning like their same aged peers when they left the Birth to 3 program.

Indicator 3: Child Outcomes - Outcome 2 (O2)

Acquisition and use of knowledge and skills (including early language/communication and early literacy)

  • Summary Statement 1 (SS1): Percentage of children who changed their growth (pattern) while in the Birth to 3 program.
  • Summary Statement 2 (SS2): Percentage of children who were functioning like their same aged peers when they left the Birth to 3 program.

Use of Appropriate behaviors to meet their needs

  • Summary Statement 1 (SS1): Percentage of children who changed their growth (pattern) while in the Birth to 3 program.
  • Summary Statement 2 (SS2): Percentage of children who were functioning like their same aged peers when they left the Birth to 3 program.

Indicator 4A: Timely Service Delivery - Rights

Percent of families reporting early intervention services have helped the family know their rights.

Indicator 4B: Timely Service Delivery - Communication

Percent of families reporting early intervention services have helped the family effectively communicate their child's needs.

Indicator 4C: Timely Service Delivery - Development

Percent of families reporting early intervention services have helped the family help their child develop and learn.

Indicator 5: Child Find Birth-1

Percent of infants and toddlers birth to 1 with IFSPs.

Indicator 6: Child Find Birth-3

Percent of infants and toddlers birth to 3 with IFSPs.

Indicator 7: 45 Day Timeline

Percent of eligible infants and toddlers with IFSPs for whom an evaluation and assessment and initial IFSP meeting were conducted in a timely manner.

Indicator 8A: Transition - Steps and Services

Percent of all children exiting Part C who received timely transition planning to support the child's transition to preschool and other appropriate community services by their third birthday including: IFSPs with transition steps and services.

Indicator 8B: Transition - Notification to LEA

Percent of all children exiting Part C who received timely transition planning to support the child's transition to preschool and other appropriate community services by their third birthday including: Notification to LEA, if child potentially eligible for Part B.

Indicator 8C: Transition Conference

Percent of all children exiting Part C who received timely transition planning to support the child's transition to preschool and other appropriate community services by their third birthday including transition planning conference, if child potentially eligible for Part B.
